- Public TitlePanelmanagement for medically unexplained physical symptoms
- Scientific TitleProactive and integrated care for patients at risk for persistent medically unexplained physical symptoms (MUPS): population care and panel management in primary care
- hypothesisPersistent MUPS may have major implications for the patient, the general practitioner (GP) and for society. Therefore it is important to early identify patients at risk for persistent MUPS and to proactively offer effective interventions to improve quality of care and to optimize use of health care services. A proactive method we use is panel management: identification of a panel of patients at risk (empanelment) based on periodical analysis of their electronic medical records (EMRs) and reporting back these patients with specific health care information to GPs as a means to identify care gaps and support interventions. The objective of this study is to evaluate the effect of panel management for empanelled patients at risk for persistent MUPS on quality of care, use of healthcare services and associated costs at the population level.
- Healt Condition(s) or Problem(s) studiedMedically unexplained physical symptoms (MUPS)
- Inclusion criteriaAdult (18-65) patients at risk for persistent MUPS will be identified and empanelled by analysis of their extracted EMRs with a validated selection method that has been developed with data mining techniques.
- Exclusion criteriacurrent or previous malignancies in the last 5 years, current or previous pregnancy in the last year and also some chronic neurological conditions

- InterventionsEvery three months, patients at risk will be empanelled and reported back with the panel management dashboard to GPs in the intervention group. The dashboard also gives information about relevant health parameters and care gaps. We will compare the effect of panel management with usual care in the control practices where empanelment and the dashboard will only be available at the background.
- Primary outcomeThe proportion of the patients at risk for persistent MUPS that are referred for a mental health care intervention, which stands for embarking on a (secondary) psychosocial intervention track annex to the somatic track.
- Secondary outcomeOverall contact frequency, total number and number of distinct ICPC codes suggestive of MUPS (appendix B), number of referrals to somatic care, number of prescribed medicines (special notation of antidepressants, benzodiazepines and pain medication), number of additional examinations, 4-DSQ subscale and total scores and panel (group) size and -change.
- TimepointsOutcomes will be measured over the preceding three months at baseline and after 3, 6, 9 and 12 months, each over a three month period.
