- hypothesisCare Sport Connectors (CSC) are employed to connect primary care, sport, and physical activity and to guide primary care patients on lifestyle programmes towards local physical activity facilities. The hypothesis is that the work of a CSC will lead to more alliance work, an increased number of residents participating in local sports facilities and being physically active in their own neighbourhood. Residents become more physically active and improve their health.

- Inclusion criteriaNine municipalities, spread over the Netherlands, are selected through convenience sampling based on contacts of project partners. The particular CSC is selected in consultation with the representative civil servant of each municipality, the total number of CSCs in the study is fourteen.
Adults who are referred by primary care to a combined lifestyle intervention or physical activity in which the Care Sports Connector is involved.

- InterventionsObservational study:
Participants of a combined lifestyle intervention or physical activity is asked to participate in the study. This means that they undergo a fitness test 3 times and after each fitness test they will fill in a questionnaire. This test is administered at the start, after their program and one year after the start of the program.
- Primary outcomeThe primary outcome measure is maintained physical activity after the completion of the exercise program or combined lifestyle intervention or not.
- Secondary outcomeThe secondary outcome measures are physical fitness (Blood Pressure - Resting Heart Rate - Height - Weight - Waist circumference - Body fat percentage - Cholesterol - Blood glucose - Arm curl - 30 second chair stand - Grip strength - 3 meter up and go - Straight leg raise - Modified Schober test - Back Scratch - Astrand endurance test or 6 minute walking test is measured with the physical fitness scan), perceived health, motivation, illness and care, self-efficacy, perceived importance, behavioral, social support, self-monitoring and exercise goals are measured with a questionnaire.
