- hypothesisFerritin is a good indicator of body iron stores. Iron deficiency and haemoglobin (Hb) deferral could be prevented among the whole blood donors with a periodic ferritin measurement.

- Brief summaryWhole blood donation invokes the risk of iron store depletion; through the haemoglobin (Hb)-bound iron loss, and subsequent increased erythropoiesis. Hb levels are frequently measured to monitor donors, but these levels do not reflect the true iron status. It is well-known that repeat donors commonly have depleted iron stores, often despite having normal Hb levels. Depletion of iron poses health risks to donors, including anaemia, but also fatigue, and cognitive impairment have been reported in donors. Depleted iron stores would eventually lead to Hb deferrals, which are costly because of wasted operational efforts and lapsing of donors. We anticipate that the implementation of routine ferritin testing in whole blood donors will result in a lower prevalence of iron deficiency and a lower risk of Hb deferrals, thereby also improving the retention of blood donors.
